For Referring Clinicians

Physician Referral Resources

Download and complete the Physician Referral Form. Fax the completed form with patient demographics, insurance information, and relevant clinical notes or pathology to (952) 520-5001. If emailing Refer@ScenicDerm.com, send patient information only through your organization’s approved secure or encrypted email system. Do not use the general website contact form to send protected health information.

Still Not Sure Where to Go?

Call the practice for help finding the right information. Current patients should use the portal or call for medical questions rather than sending protected health information through a general website form.
