Download and complete the Physician Referral Form. Fax the completed form with patient demographics, insurance information, and relevant clinical notes or pathology to (952) 520-5001. If emailing Refer@ScenicDerm.com, send patient information only through your organization’s approved secure or encrypted email system. Do not use the general website contact form to send protected health information.
