Online check-in, needed consents, and paperwork may be completed through the portal before your appointment.

Protect your information: Do not enter a portal username, password, date of birth, or medical information on this Scenic Dermatology page. Enter portal credentials only after the external Sadio website opens.

Complete Available Check-In

Complete available check-in, needed consents, and paperwork in the portal. If these are not completed online, they can be completed on a check-in iPad when you arrive.

  • Complete available online check-in before your appointment.
  • Sign needed consents.
  • Complete needed paperwork.
  • Depending on availability and your portal access, certain records or documents may also be viewable through the patient portal.

Release of Information Authorization

Use this form to authorize Scenic Dermatology to send specified medical records to another person or organization, or to request records from another provider or facility. Completing the form does not provide immediate access to records; availability and processing depend on the request.
